PSUN282 Sertraline-Induced Hypoglycemia in a Non-Diabetic Patient

Abstract Background Sertraline is one of selective serotonin reuptake inhibitors (SSRIs) commonly prescribed for depression. Irrespective diabetes status, SSRIs can cause glycemic dysregulation. Clinical Case 74-year-old man with depression was evaluated hypoglycemia. His managed sertraline more than 10 years. He did not have diabetes. Non fasting, laboratory studies done during annual evaluation showed blood glucose 29 mg/dL but he didn't any symptom hunger, sweating, shaking or dizziness. Lab report initially considered to be an error. Repeat 102 mg/dl. Subsequent labs 6 months later also reported 27mg/dl. suffered from a bicycle accident head laceration in May 2021, his on presentation ED 38 mg/dL). Follow up fasting 86 mg/dL, insulin level 4.2 IU/ml (1.9-23. 0 IU/ml), c-peptide 1.3 ng/ml (0.8-3.5 ng/ml), AM cortisol 22.6 mcg/dL (6.7-22.6 mcg/dl) and hemoglobin A1C 5.3%. Thyroid function test normal. discontinued continuous monitoring which there were no hypoglycemic episodes. The exact mechanism sertraline-induced hypoglycemia remains unclear.
